How Do I Access my Patient Portal?

  1. When you are first web-enabled, you will receive an e-mail that looks like this:patient-portal-1
  2. Click on the URL in this e-mail to get to your portal. Your log-in screen will look like this:patient-portal-2
  3. If you have a message waiting in your portal, you will receive an e-mail that looks like this:patient-portal-3
  4. If you are locked out of your account or need to reset your password, please call our office.
  5. Once your account has been unlocked and/or your password has been reset, please wait 15min before you try to log in to your account again.
  6. If you are not web-enabled and would like to be, please call our office to get set-up.
